Patients may request access to their medical records in accordance with relevant legislation. Requests should be made in writing and addressed to the Practice Manager. Proof of identity may be required before access is provided.
If you wish to transfer your medical records to another healthcare provider, please complete our Medical Records Transfer Request Form and provide written consent.
A reasonable administrative fee may apply for the preparation and transfer of medical records where permitted by law. Records will be transferred securely to your nominated healthcare provider within a reasonable timeframe after your request has been processed.
